New Applicants must have the Following:

1.  Completed CCW Application form (Find Below)
2.  Valid Photo ID ( Driver's License)
3.  Recent colored head to shoulders passport style               photo (2x2.5) 
4.  Original Certificate of completion of the Firearms course signed by an approved Ohio instructor
$67.00 CASH if you have lived in Ohio for the last five years
$85.00 CASH if you have not lived exclusively in Ohio for the last five years

This is a list of OPOTC and NRA certified instructors who have provided their teaching certifications and, in most cases, their telephone numbers. It is the applicant's responsibility to check the instructor's certification and ensure they are teaching according to Ohio state law.
